What Conditions Are Treated With Epidural Steroid Injections?

Epidural steroid injections are commonly used to treat inflammation and nerve irritation caused by spinal conditions. They can help relieve symptoms such as radiating pain, numbness, or tingling in the back, neck, arms, or legs.

Conditions commonly treated include:

  • Herniated or bulging discs
  • Spinal stenosis
  • Sciatica
  • Degenerative disc disease
  • Radiculopathy (pinched nerve)

What are the different kinds of ESI injections?

Epidural steroid injections (ESIs) are categorized by where they’re given along the spine (cervical, thoracic, or lumbar) and by the needle’s approach to the epidural space, such as interlaminar, transforaminal, or caudal.

The three main types include:

Cervical (neck) ESI – The injection is placed just to the side of the spine in the neck region. This is often recommended for neck pain that radiates into the arms due to conditions like a herniated disc, bone spurs, or spinal stenosis.

Thoracic (upper and mid-back) ESI – The injection is placed slightly to the side of the spine in the upper or middle back. It may be used for pain caused by a thoracic disc herniation or spinal stenosis in this region.

Lumbar (lower back) ESI – The injection is placed just to the side of the spine in the lower back. It is commonly recommended for lower back and leg pain related to lumbar disc herniation, degenerative disc disease, or spinal stenosis.

When Should You Consider An Epidural Steroid Injection?

An epidural steroid injection may be recommended when conservative treatments, like physical therapy, rest, or medications, haven’t provided enough relief. It’s typically considered for those experiencing nerve-related symptoms that interfere with daily activities or quality of life.

  • Radiating pain from the neck or lower back into the arms or legs
  • Persistent nerve irritation due to a herniated disc or spinal stenosis
  • Difficulty standing, walking, or sitting for extended periods
  • Inflammation not responding to other treatments
  • A need to delay or avoid surgery while exploring other options

What Happens During an Epidural Steroid Injection Procedure?

During an epidural steroid injection (ESI), you’ll lie on your stomach or side while the skin over your back is cleaned and numbed with a local anesthetic. Using live X-ray guidance (fluoroscopy) for accuracy, your provider carefully inserts a thin needle into the epidural space or the area surrounding the spinal cord and nerves. Once the needle is in position, a small amount of contrast dye may be injected to confirm correct placement.

Next, a combination of corticosteroid (to reduce inflammation) and anesthetic (to help with immediate discomfort) is delivered into the epidural space. The entire procedure typically takes less than 30 minutes, and you can usually go home the same day. Relief from the numbing medicine may be felt quickly, while the steroid’s effects may take a few days to fully develop.
